How to Request a Leave of Absence

Report a new leave, intermittent absences related to an existing leave, or verify the status of a previously-reported leave. Customer service representatives are available to assist you, Monday through Friday (excluding major holidays).

Step 1

Notify your manager of your request and contact The Hartford customer center at 888.980.9382

Please have the following information available:

  • Your Aerospace ID number
  • Policy #: 402802
  • Your treating physician’s name, address, phone & fax numbers

Follow the prompts to report a new leave or changes to your existing leave.

Step 2

Submit Your Request

Before registering, you must submit your request for leave.

  • Select the type of claim on the upper left side of the page
  • Enter your email address
  • Enter policy #402802

Step 3

Register now

  • New users must register online or via the Hartford’s Mobile App by selecting the “Register Now” option to create a secure unique username and password.
  • You’ll receive an email confirming your registration.

Once your registration is complete and your account is set up, you may log in at any time to view any existing leave requests, or to request a new leave.
